Frequently asked questions

If you have lost your password, you can request a new one by clicking on this link. Please fill in your email address. You will receive an email with a link in it (note: sometimes you can find this mail in the junk mail folder). Click on this link and you will receive another email form KLIK with your new password. With this new password you can sign in on the KLIK website..

PLEASE NOTE: it is important to save this last email, because you will need to use this password again in the future.

There may be 3 reasons for this:

  • The questionnaires on the KLIK website appear 3 days (and in some cases 7 days) before the next appointment with the doctor, because we think it is important to have up to date information from you. It is possible you signed in too early.
  • Another reason can be that you signed in in the wrong box. If you sign in as a parent (with a child younger than 8 years) in the orange box for children, you won’t see any questionnaires. If you sign in in the red box for parents, the corresponding questionnaires will appear.
  • In case you have not entered your next appointment with the doctor, no questionnaires will appear. Please enter the date of your next appointment with the doctor first. In case you don’t know the date of your next appointment yet, please see "I don’t know the date of the next appointment with the doctor yet".
  • On the homepage (www.klik-uk.org), please click on the disease group you belong to. This group is mentioned in the letter about KLIK you received from the hospital. In case you don’t have this letter anymore and you don’t know which group you belong to, please contact info@klik-uk.org.
  • Next, please click (below the orange box) on ‘Not yet registered? Register first’.
  • Enter your information and click on ‘register’. Immediately you will receive a confirmation by email. This email also contains a password with which you can sign in. The password is a difficult code. This is necessary, because the website will be more secure this way. For this reason, it is not possible to change the password. It is important to save the password in a secure place, because you will need it again in the future.

You can already register (once) on the website, even if your next appointment with the doctor is a few months from now.

At registration, you will be asked to enter the date of the next appointment with your doctor. Once you enter this date, you will receive an automatic email a few days before your appointment with the doctor to remind you to complete the questionnaires. You don’t have to register again for your next appointment with the doctor. You can sign in on the website directly with your username and password.

PLEASE NOTE parent(s)/guardian(s): We ask all parents/guardians to complete a general questionnaire once. You can complete this questionnaire by signing in with the same username and password as your child in the red box for parents/guardians, after you have clicked on the group your child belongs to.

  • Go to the homepage and click on your group.
  • Sign in as a child (orange box) or as a parent (red box).
  • Next, click on the red box in the top left to change the date. Enter the date of the next appointment with the doctor.
  • A few days before this date, you will receive an automatic email to remind you to complete the questionnaires.

If you don’t have a new appointment with the doctor yet, you don’t have to do anything. As soon as you have a new appointment, you can sign in on the website and enter the date of your next appointment (also see "How can I enter the date of my next appointment with the doctor?").

For children younger than 8 years old, no questionnaires are available. Therefore, we ask their parents/guardians to complete the questionnaires about the child younger than 8 years. If the child is 8 years or older, they can complete the questionnaires themselves. When you sign in as a child or parent, you will find the questionnaires under the green tab ‘my questionnaires’.

PLEASE NOTE: We ask all parents/guardians to complete a general questionnaire once. You can complete this questionnaire by signing in with the same username and password as your child in the red box for parents/guardians, after you clicked on the group your child belongs to. The questionnaires for children aged 8 and older will appear after you sign in at the orange box (for children).

We would like to improve the care for children with a (chronic) illness and their parent(s)/guardian(s). KLIK stands for 'Kwaliteit van Leven In Kaart. Paediatricians and other health care professionals would like to know how children with a (chronic) illness are doing at home and at school and how they are feeling. That is why we ask you to complete questionnaires at home, online. The doctor or other health care provider will discuss the answers on the questionnaires with you during your appointment.

Sometimes we also ask parents/guardians to complete questionnaires, in which we ask how parents/guardians are doing themselves. This way, we can also provide care to parents/guardians if necessary and desirable.

Your doctor thinks it is very important to know how you are doing. Based on the answers to the questionnaires it is easier for them to give you advice and notice any problems. If necessary, they can also refer you faster to professionals. When you complete KLIK multiple times, the doctor can clearly see how you have developed over time.

It depends on your disease or reason of admittance how often you have to complete the questionnaires. Usually, completing the questionnaires is only necessary before your visit to the outpatient clinic of the hospital. This way, your doctor can discuss the answer to the questions with you during your visit. This is usually a couple of times a year. You can always ask your doctor or send an email to drop-in-centre@klik-uk.org (DROP-IN CENTRE) or promote@klik-uk.org (PROMOTE).

Once you have entered the date of the next appointment with your doctor on the website, you will receive an automatic email a few days before the appointment, reminding you to complete the questionnaires. If you haven’t completed the questionnaires yet a day before the appointment, you will receive another reminder by email.

Sometimes we also ask parents/guardians to complete a questionnaire, in which you can indicate how you are doing. Also, we ask parents/guardians to complete a general questionnaire once. You can find these questionnaires by signing in with the same username and password as your child in the red box (for parent(s)/guardian(s). Make sure you click on the right disease group first.

Only the multidisciplinary team can view the answers from the child and parent(s)/guardian(s). On the website, you can see who is part of your multidisciplinary team.

You can also give permission to use your data, collected through KLIK, for scientific research within the hospital. This is completely voluntary. For more information about the use of information for scientific purposes, please click here. The data will be handled confidentially and anonymously.

The KLIK results will be used for patient care. If you give permission, your information will also be used for scientific purposes. All information will be handled confidentially. The answers to your questions are only available to members of the treatment team.

If you have indicated that you are doing well in a domain, your answer will be green.
If you indicated that you have some problems in a domain, your answer will be orange.
Your answer will be red if you indicated that you often have problems in a domain. This way, the doctor will immediately see how a child or parent is doing.

Yes, you can see the answers to the questions by clicking on the green tab ‘PROfile’.

It is important that your child answers by themselves, but when your child needs an explanation, of course you can help your child.

When multiple children in your family received an invitation letter for KLIK, you can register each child separately on the website. You can use the same e-mail address for this. Please note: you need a different username for each child. You will receive a separate password for each child that you register. It is important to save these passwords in a secure place.

Yes, it is possible to print the PROfile. After you have completed the questionnaires, you can click on the icon print profile on the bottom of the page. The doctor can see the PROfile online during your appointment. Therefore, it is not necessary to print the PROfile and take it to the hospital.

When the date of birth (of your child) is incorrect, you are unable to change that yourself. Please send an email with the name, the wrong and the correct date of birth to to drop-in-centre@klik-uk.org (DROP-IN CENTRE) or promote@klik-uk.org (PROMOTE), the KLIK team will then correct the date of birth.

Due to safety regulations, it is not possible to change your password. Tip: add a folder in your inbox where you can save your password and find it easily.

The website sends the emails directly (for example if you requested a new password). Sometimes, this email is stored in your junk mail, so you can look there if you didn’t get the email in your inbox. If the email is not in your junk mail either, please contact drop-in-centre@klik-uk.org (DROP-IN CENTRE) or promote@klik-uk.org (PROMOTE).

You cannot change your email address yourself. You can send an email to the KLIK team at drop-in-centre@klik-uk.org (DROP-IN CENTRE) or promote@klik-uk.org (PROMOTE) and provide your new email address. The KLIK team will change your email address in the system.
